On February 15, 2023, the FAA published Notice N8900.651, which introduces updated forms and policy regarding airman applications made under Part 65 Subparts D, E, and F. The Notice discusses the following forms:

The notice, directed at FAA personnel, can be found in the Dynamic Regulatory System (DRS) at https://drs.faa.gov or on the FAA’s website at https://www.faa.gov/regulations_policies/orders_notices.
Applicants who have already started the certification process (to include the IA renewal process) using a prior version of a form may continue to use the prior version in most cases. FAA personnel will direct the applicant to complete a new form only if necessary to complete the certification process. Below is a summary of the changes made to each form:
