IFR Practice Approaches & Recent Pilot Deviations

We have seen a serious uptick recently in pilot deviations where pilots fly an instrument approach under an IFR flight plan, presumably for training, then go missed, then start turning towards their next destination and climbing to an altitude of their choosing without ever having canceled   their IFR flight plan. 

As you know, an instrument approach clearance under IFR permits the pilot to either land or fly the published missed approach procedure, assuming no alternate instructions have been issued or coordinated.
