Good Intentions + Lack of Knowledge = Fatal Consequences   Substituting a recommended compound with a non-recommended compound can have unintentional consequences. An example of this is when an operator substituted a lubricant with a non-lubricant during an installation of a threaded rod end. The procedures only called for grease on the threaded rod end. The operator’s intent was to enhance corrosion protection but instead he unknowingly affected the preload on the threaded connection by using the torque value for threads with lubricant. Due to the increase of runway incursions at Van Nuys Airport, John Wayne Orange County Airport, and the Chino Airport, educational videos have been produced to make pilots more aware of the problem areas at each airport. Advocates for Aviation Safety

The Advocates for Aviation Safety has recently become an Industry Member.  Their focus is entirely on helping us promote the WINGS program.  Paul Burger, the Advocates founder, has recently put up $10,000 to create the CFI Sweepstakes Challenge.  CFIs get one entry into the sweepstakes drawing each time they get a pilot phased in WINGS .  This year, CFIs must be NAFI or SAFE members to participate.  But, next year, it will be open to all CFIs.

Monthly Report As of: 11 December 2018 The USHST is a regional partner to the International Helicopter Safety Team. USHST GOAL: Reduce the fatal U.S. helicopter accident rate by 20% by 2020. New Safety Video for Tucson International Airport

Numerous wrong-runway landings have occurred at Tucson International Airport (KTUS), especially when runways 11/29 are in use.  Many pilots have mistaken runway 29R for 29L, 11L for 11R, or Taxiway Alpha for a landing surface. Taxiway Alpha is north and parallel to Runway 11L/29R. Pilots are urged to thoroughly plan for their arrival at Tucson in advance. Review the airport diagram, as the airport geometry is complex. “20% by 2020” Monthly Report (July 2018) The USHST, a regional partner to the International Helicopter Safety Team, set a goal in 2016 of reducing the fatal U.S. helicopter accident rate by 2020 to 0.61 fatal accidents per 100,000 flight hours, compared with USHST’s baseline rate of 0.76 per 100,000 hours. That number is the average fatal accident rate for 2009–2010 and 2012–2014.  The fatal accident rate has been trending downward over the past 15 years, but it has been below 0.61 only twice, and experienced a sharp increase in 2008 and 2013.
